Barbell or DB?
I use a barbell with my hands about 4 inches apart.
bb, hands just inside where the gnurling starts
great way to fuck your wrists up

really? They haven't hurt me at all - yet. I'm up to like 225 for a couple reps, maybe when I increase poundage..?
and bring your elbows forward, not out. (more akin to the powerlifting form of benching)

elbows tucked on the eccentric phase, flared on the concentric phase
if you flare them you put more stress on the lateral head compared to the long head.
