Is it something they do during pageload like in PHP or something? or are they doing it somehow at the server level like in an htaccess?

doing a site now that will have city-specific subdomains.. so it would be nice if the city-specific subdomain was selected automagically (if applicable) when the user visits the primary site

Most use MaxMind GeoIP Web Service through javascript to return the Country, Region, City, Latitude, and Longitude of the web visitors and others are custom made.

MaxMind's API is free. Google it.
