I always start my games on hard. I like a challenege, most games are too easy these days.
I usually go hard or the hardest depending on the game and how many times I want to play it
Start on hard, if it's too much for me I usually go back to Medium. I've only had to do that once in the last year though, with MGS4
the Medium level generally. I like to get through games but also have somewhat of a challenge.
hard and i also play with my eyes closed
The default setting because lets be fair im likely only to play it once or twice
normal/medium then if i really dig it i may move to hard.
Usually the medium/normal setting is best. I just want to get through the story and enjoy the gameplay. Usually normal isn't too easy or hard.
usually medium. then later ill play through it again on the hardest level for extra play value.
easy/medium, depending on how much story it has so I can enjoy that instead of starting from the same check point 15 times. Then on hard with all the knowledge.
normal/med or hard depending on the type of game, i tend to suck at fps on consoles so i go med on those if its a platformer, hard.
btw, i hate how some games require you to unlock hard mode, the most recent example i remember is wolverine origins, i breezed through the game on normal, unlocked hard, by then i lost interest. probably would of enjoyed the game more on hard.
Try MGS4 on The Boss Extreme, no kills/alerts/continues, using no health items.
Shit is hard, but so fulfilling if you can manage to do it. I'm up to the motorcycle chase so far.

normal. I started Halo 3 on Legendary cause I had experience
easy or default
Difficulty settings are starting to get annoying. If I'm not sure what I'm getting in to, I pick normal because I assume that's what the developers recommend. The only recent FPS I really had fun with on hard mode was Rainbow Six Vegas.
usually one up from default, I like the challenge.
anybody else hate when the increase in difficulty just lowers damage and raises enemy defenses
I also hate it when normal is too easy and hard is too hard.
default, unless it's a game like inFamous that suggest difficulty and then I usually start on that. inFamous suggested I play on the hardest level.

med-hard depending what type of game. most of the time i start at medium if i feel like beating again ill start at the hardest mode possible.
like medium dead space to impossible
or medium to extreme in mgs games.
As hard as possible, and before I start I like to slice my fingertips up with paper cuts and splash chlorine in my eyes so that it is actually a challenge...
